Financial Assistance Policy Summary

The Pathology Laboratory Inc is committed to meeting the healthcare needs of the communities we serve. Medical care is provided to everyone, regardless of ability to pay or lack of insurance coverage. Financial assistance is available to help eligible patients who cannot afford to pay for their essential healthcare services.

Our financial counselor can help you understand if you qualify for Medicaid or other low-income programs offered by the State. Once it is determined you do not qualify, the laboratory will assist you with The Pathology Laboratory Charity Care and Financial Assistance Program.

Eligibility & Discounts

Participation is voluntary and based on household income and size relative to the Federal Poverty Level (FPL):

Household Income	Discount Amount
Below 200% of FPL	100% Charity Discount
200% to 300% of FPL	Sliding Scale Reduction
300% or Greater of FPL	Minimum 74% Discount

Required Documentation

The following documents are required to apply:

Proof of income for the last 30 days (pay stubs, Social Security award letter, etc.)
Last filed federal income tax return
Copy of a Medicaid denial letter
