Financial Assistance Policy Summary

The Pathology Laboratory Inc is committed to meeting the healthcare needs of the communities we serve.  Medical care is provided to everyone, regardless of ability to pay or lack of insurance coverage.  Financial assistance is available to help eligible patients who cannot afford to pay for their essential healthcare services.  The Financial Assistance Policy describes who is eligible for help, what services are covered and how to apply.  This document summarizes that Policy.

Our financial counselor can help you understand if you qualify for Medicaid or other low-income programs offered by the State.  Once it is determined you do not qualify, the laboratory will assist you with The Pathology Laboratory Charity Care and Financial Assistance Program.

You may be eligible for help with your medical bills if you are uninsured and have a low income.  Participation in the program is voluntary and based on household income and size.  For Laboratory services, eligible families that have incomes below 200 percent of the Federal Poverty Level will receive a 100 percent charity discount.  If your household income is between 200 to 300 percent of the Federal Poverty Level, a sliding scale will be used to calculate your reduction of charity discount.  Household income at or greater than 300 percent of the Federal Poverty Level qualifies for a minimum 74 percent discount in billing.  Discounts will be applied to gross charges.  Eligible individuals will not be charged more than amounts generally billed.

The following documents are required to apply for the Indigent/Charity Care and Financial Assistance Program listed above:

  • Proof of income for the last 30 days (pay stubs, Social Security award letter, etc.)
  • Last filed federal income tax return
  • Copy of a Medicaid denial letter.
